Initial Intake Meeting
We are a mobile team which allows us to work with participants and providers in every corner of the state
Your allocated Specialist will meet with you at your home, treatment facility or preferred location. Virtual meetings are also offered for those who like to connect with more flexibility.
During this meeting, your specialist will work with you to develop an individualised support plan that reflects your personal goals and needs. They will also work with you to connect you to various providers as needed
